Background: Sepsis-induced acute kidney damage (SI-AKI) is a major health concern that causes a lot of death and disability. Long non-coding RNAs (lncRNAs) play a crucial role in sepsis by regulating inflammatory responses and organ failure. One such molecule is taurine upregulated gene 1 (TUG1), which plays an important regulatory role in SI-AKI. The purpose of this research was to examine the levels of lnc-TUG1 expression in the plasma of SI-AKI patients and assess its biomarker's potential. In this case-control study. Fifty patients who have SI-AKI (Sepsis-3 criteria), Kidney Disease Improving Global Outcomes (KDIGO)-defined (AKI) within 72 hours, and fifty healthy participants served as healthy controls. Plasma samples were taken within 24 hours after ICU admission, plasma lnc-TUG1 was measured by RT-qPCR. SI-AKI patients' plasma lnc-TUG1 expression was noticeably lower than that of controls' (p < 0.001). Clinical indicators such as SOFA score (r = -0.569), IL-6 levels (r = -0.550), and APACHE score (r = -0.488) (p < 0.001) in all showed significant negative relationships with lnc-TUG1 expression. An area under the curve of 0.808 (95% CI: 0.740-0.925) was shown by the receiver operating characteristic curve analysis to indicate that lnc-TUG1 has strong diagnostic capability for SI-AKI. The optimum threshold for lnc-TUG1 was 0.560, and at that point, it was 92.0% specific and 86.0% sensitive. Our results indicate that lnc-TUG1 is downregulated in SI-AKI, which raises the possibility that it might be used as a biomarker to assess the severity and outcome of kidney failure caused by sepsis.
